I have learned and utilize the Dynamic Temporal and Tactile Cueing (DTTC) Approach, which is an evidence-based treatment for patients with suspected and confirmed CAS. This technique involves intensive trial with a small number of target words, that are relevant and important to the child. This treatment requires practice with increased repetition. Additionally, DTTC focuses on other elements including prosody and volume. The parent is actively involved in the selection of target words to ensure that the words are motivating for the child. DTTC utilizes a cuing hierarchy. Additionally, I have also trained in Physical Restructuring of Oral Motor Phonetic Targets (PROMPT) where the clinician will use his or her hands to support the motor movements as the client practices saying those words. Neither of these therapy modalities are used exclusively and I often incorporate both within sessions.
Parent involvement is critical for progress. It is extremely helpful to get parent input on targets. Home carryover practice is a crucial element so providing parent education and assistance is an extremely important aspect of treatment.
I utilize both low and high tech AAC to aid in communication and decrease frustrations. The fewer breakdowns that occur the more time available to target selected words.
