My therapy for a child with CAS is fun but structured. I play off of the child’s interests to provide engaging play based therapy. Play-based therapy could include playing with toys on the floor, playing a game at the table or using gross motor tasks (swinging, bouncing, etc.) to encourage communication. I work with the family to develop functional targets that will help with communication across settings (home, school, community, etc.). I use various levels of cueing to assist with speech sound production including verbal cues (i.e. “lips together”), touch cues (PROMPT cues), visual cues (Easy Does It consonant and vowel cues), Using the DTTC method, I will simultaneously produce the utterance with the child and will encourage self-reflection through video or audio recording. I will have the child produce each target 3-5 times to increase accuracy of production. I will work with the family to determine appropriate approximations/simplifications for more challenging words. We will work on family established core vocabulary as well as clinician directed sound combinations. We will start at the vowel/consonant level and then shape the sounds into more complex sound combinations such as consonant-vowel/vowel consonant combinations. I will teach the family to use some cueing methods so that they can carry over what the child is learning to the home environment. AAC will be incorporated as needed through low tech (picture communication systems) and high tech (speech generating devices) modalities. Families will be involved through each step of the process.
My sessions include parent discussion at the beginning and end of the session. This discussion will include any new concerns and review of the home exercise program. I will work with the family to determine if having them in the session is beneficial. The family and I will establish functional targets that will help with communication across settings. Homework will be given for the family to practice outside of sessions. I will provide homework in the form that the family prefers (written or digital) that is specific and will carry over what was taught in therapy. The family will be connected to outside resources that will encourage them in their journey of having a child with a CAS diagnosis.
Parents and I will work together to implement AAC early in therapy. We will develop functional vocabulary lists and build photo/icon single pictures or communication boards that will supplement spoken language. High tech options will also be considered in collaboration with the parents. We are able to acquire these devices through insurance or I can use a pre-established device in therapy. These tools will be incorporated in therapy and parents will have the opportunity to see the AAC in action and ask questions.
