My therapy process starts by determining and describing exactly what is happening with a child's speech so I can apply the most effective treatment strategy. When treating CAS, I use a motor-learning approach. This means determining the syllable level at which the child's speech is breaking down, and focusing on those motor movements. Therapy begins by providing as much support as necessary (e.g. saying the words simultaneously together, or with multisensory cues) for the child to accurately execute the continuous motor pattern. There is a focus on producing as many accurate trials as possible within each session, and gradually increasing the number of different functional words practiced. As the child becomes more proficient, the level of prompting and cueing is reduced by using variations of imitated productions and time delayed responses, to move toward spontaneous productions. Feedback given to the child also begins as frequent and specific, and then fades.
Parents are involved in many parts of the therapeutic process. As the experts on their child, parents know what words/phrases/sentences are most meaningful to them. Incorporating those messages can be very motivating for kids! Each treatment plan has cooperative elements for parent input. During therapy sessions, parents are encourage to observe and participate. Training in prompting techniques empowers parents to incorporate elements of therapy throughout their child's day!
When children are unable to communicate their needs, thoughts, and feelings verbally, augmentative and alternative communication (AAC) can benefit them. As an AAC specialist, I support families who are open to supporting their child by using communication boards or speech generating devices. This reduces frustration and allows for expressive language development. It also provides a consistent speech model.
