In early sessions, I focus on building rapport with a child and educating and counseling their caregivers on CAS. I establish initial objectives for therapy based on the child's current speech skills and the family's priorities. My approach is based on a variety of accepted motor-speech interventions and uses an array of verbal, visual, and tactile prompts in an effort to find out what a child responds best to. Typically, sessions are play-based with a mixture of child-led and adult-led activities. Play will be structured in such a way as to maximize opportunities to imitate a small set of targets, which may consist of sounds, syllables, or words, based on a child's speech repertoire. As a child masters targets, I will add in utterances of increasing complexity, slowly working toward complete and fluent conversational speech.
I believe it is very important for caregivers to feel confident in their understanding of their child's diagnosis, as well as how they can support their child's speech development and communication overall. I encourage any and all caregivers to sit in or observe therapy sessions in order to see first hand the strategies I use. If caregivers are unable to sit in or observe sessions, I dedicate the last 10 minutes of the session to debriefing with them and going over how to practice targets at home. I like to keep open lines of communication regarding a child's progress and objectives and am happy to schedule additional time with caregivers to go over their questions and concerns.
I feel strongly that every child should have the ability to express their wants, needs, feelings, and opinions. I practice a Total Communication approach to therapy, honoring all modes of communication including vocal, sign, written, and visual. I have used a variety of AACs ranging from PECS/communication boards to SGDs including Proloquo2go, TouchChat, and LAMP. I work with families to determine which AAC may be most useful for their child and what role it will take in their communication journey.
