While every session is unique to the child's interests and work ethic, one thing they all have in common is high intensity treatment masked with fun! The Kaufman Speech to Protocol is utilized in every CAS session to aide in producing various syllable word shapes including reduplicated CVCV, CVC, VCV, etc. while reinforcing their best approximation of the words. Vowel and consonant repertoire and error patterns are examined to determine where to begin in treatment and establish targets. Treatment approaches include the following: verbal and visual cueing, fading of cues based on the child's accuracy, errorless learning (cueing before failure), block to random practice, and varying stimuli to avoid overgeneralization. Children with CAS often have difficulty with more complex words, the beauty of the K-SLP method is to simplify words to the child's best approximation or the most age appropriate phonological process. By helping them simplify words, they are able to label and request more often using a vocalization. If vocalizations are difficult to produce, sign-to-talk is implemented as a bridge communication for minimally vocal children. Overall, each session varies based on the child's abilities to participate, interests, amount of support needed.
Parents and caregivers are able to watch every session from a one-way observation window. Following each session, I leave a few minutes at the end of the sessions to discuss what went well, what was difficult, and what should be worked on at home. I educate the parents on goals targeted and treatment approached used. I answer any questions in-person, and always provide my e-mail and phone number to answer questions at a later time.
AAC has been utilized in my sessions in a variety of ways including modeling throughout the session while verbally navigating, targeting goals for requesting and labeling using prompts, pointer cues, and direct models. Reinforcement is provided when the child appropriately requests/labels to ensure consistent use of device will be completed.
