My sessions are fast-paced and incorporate numerous practice opportunities with lots of fun and play. First I establish a rapport with the child, using highly motivating reinforcers, with input from parents, to get consistent participation, increased number of trials and success. I use The Kaufman Speech to Language Protocol (K-SLP) an evidence-based treatment approach, rooted in the principles of motor learning. Treatment targets are chosen based on the child's repertoire of consonants, vowels, syllables and error patterns. Parents are asked to provide a favorites list of words which are evaluated for production stimulabilty and used as first vocabulary words. Early learner words are chosen based upon functionality, high frequency, power words (up, in, go, etc), favorites list, common nouns and verbs and specific speech targets. Words are simplified, according to phonologcal rules, to the child's best level of approximation. As the child's speech motor skills improve, closer approximations are taught, reinforced and expected, moving to the adult form of the word. The speech motor hierarchy progresses from syllables to words, to phrases, to sentences in structured tasks to conversation. Numerous practice opportunities are provided from simple structure to conversational speech, as target vocabulary moves into functional expressive language. Treatment methods include: providing as much multisensory cues as needed, fading cues based upon child's performance, strategic reinforcement, errorless teaching, error correction, block to random practice, mass to distribute practice, mixing and varying tasks to avoid teaching overgeneralization. Parents observe each session and are coached on home practice to strengthen speech and language development in play and in the natural environment.
Parents are encouraged to observe each treatment session. Specific suggestions, cues, scripts and strategies are modeled and discussed during and after the session as to how to work with their child in play, with common routines and in the natural home environment. Suggestions are given as to how to set up the home environment to optimize progress and the home program. At times, parents are brought into the session to work side by side with me. The Kaufman Speech Praxis Workout Book handouts and other materials are provided to strengthen the home program and for parent education. Teletherapy provides a view into the child's home and natural environment. Parents are coached in how to set up the home, play routines, strategies and scripts to strengthen and generalize speech and language development.
I am not trained in AAC devices. I will refer to a consultant if the child is a candidate for an AAC device as the primary means for communication. I have experience with using PECS. I use sign language as a bridge to vocal communication skills, when needed.
