My therapy sessions look a lot like play. I believe in following the child's lead to see what is interesting and motivating to them. Once the child picks the activity for the session, it's my job to figure out how to build in the targets and multiple repetitions. I like to work with parents throughout the session and explain what I'm observing and how I am grading the amount and type of facilitation I'm providing to help the child be the most successful with their targets, with the least amount of facilitation from myself. I also work with parents on their understanding of how to produce the target sounds/words so that they are reinforcing the correct motor plan outside of therapy. I've found that parents videoing my models with a variety of cues they can use at home to be beneficial. I try to focus on functional communication goals. To do this, I work with the family to determine what words or targets will be the most meaningful and impactful for their child to communicate. Depending on the child's communication needs, I might introduce or model with AAC throughout the session.
I like for parents to sit in on all therapy sessions and participate in learning how to model and facilitate correct and accurate productions of target words for them to carryover outside of therapy sessions. I also think it is important for parents to have a say in the direction of therapy and chose target words that are meaningful for their child.
I like to introduce AAC immediately into therapy sessions. First by modeling for the child and providing Aided Language Input, then by working on the child using the AAC device to communicate their wants and needs. My preference is to use high tech, speech generating devices with an app that uses motor planning. As the child demonstrates interest and use of the device, I discuss with families when we should move forward with completing an AAC evaluation to try and obtain a personal device for their child through insurance.
