Within our practice, we offer different options for the child and family. Some children are seen in an intensive model, three to four times a week. Most of our children are seen once or twice a week with intensive parent training for carryover at home. I focus on a child’s ability to functionally communicate his/her wants and needs to caregivers. My therapy approach combines a fun and engaging environment for learning. I focus on developing trust and a safe space and place for practicing what is very difficult for many children.
I use a combination of the Dynamic Temporal and Tactile Cueing approach to support a child’s ability to acquire new movements for speech production and the use of adapted Motor Speech Treatment Hierarchy to guide target selection. Our target selection is different for every child. I focus on various consonant-vowel shapes, phonemic targets, rate and prosody, coarticulation transitions between sounds and varied multisyllabic syllabic levels. I also use the chaining and cycles approach contingent upon the individual child’s needs and what approach provides the most success for intelligible speech production.
Caregivers and providers are the most important window to the ability of the children we serve. They serve as the informants for their child’s abilities and development and progress outside of the therapy space and in their natural environment. Direct observation and training via video modelling, as well as direct parent training in the natural environment are a part of each client’s plan and program. We collaborate with teachers and other professionals that are part of our children’s plan.
I offer both low tech and high tech AAC options are used in our clinic. I will typically evaluate and determine the best AAC option for the client with support from AAC specialists on our staff. It has been my experience that a dedicated device is most successful for most of the children that I treat. I believe in using a global approach in that we follow the child’s lead. The goal of my therapy plan is to give each child a voice, and this is when AAC becomes most valuable, even for some of our youngest children. I incorporate the chosen AAC talker during play activities and storybook to teach the function of communication and to empower children. This does not take the place of a motor planning approaches, but enhances the process and often helps decrease frustration and increase verbal attempts.
