When treating children with CAS, I think building rapport is the first step in successful therapy. When a child is comfortable and motivated with their therapist, this is when you will see the most progress with their individualized goals. Based on a thorough motor speech evaluation and family input, I create functional goals that are important in the child's day to day life. I encourage parents to be present in my sessions so they can understand the techniques I am using and imitate these strategies at home for practice. Treatment sessions are centered on the motor learning principles repeated, correct productions with specific feedback on performance. Utilizing the motor-speech hierarchy, I begin with syllable shapes to approximate meaningful words with increasing the difficulty of targets. Depending on the child, a variety of cues including verbal, visual, gestural, and tactile will be used to best suit each child's needs.
I encourage parents/caregivers to be involved with therapy from our initial interaction. Parents are able to provide core vocab targets which influence therapy activities. Following therapy sessions, I want parents to feel comfortable with the expectations of modeling, practice targets, and techniques to utilize at home.
I have implemented AAC use (both low and high tech) for children with CAS. Use of AAC should be sought from the beginning of therapy to help provide all children with a voice when verbal speech is difficult. I have experience trialing a variety of communication applications to determine which is best for a child and then help parents advocate for these devices.
