What I love about my work with children who have apraxia is that they are bright, able children who require us to think outside the box! As a bilingual clinician, I am encouraged to amp up our level of 'fun' in sessions and approach treatment in a way that is functional, achievable but challenges the client and their family to work towards more. My families and I work towards facilitating communication that is effective and efficient so that the client is able to communicate their needs/wants across all natural settings and with all speakers.
Our sessions allow the child to lead in play either at the table, in the playroom our even outside. I believe children grow and use their words when the "motivation meter" is at its highest level; this means following the child's lead in sessions along with providing choices for activities and intertwining novel more challenging tasks as we move forward in treatment. My typical session might include implementing the child's target words through a gross-motor activity (obstacle course, for example), songs, pretend play activity, picture book or board game. Targets are carefully chosen and may focus on phonatory control (air-breath), mandibular control (jaw), labial control (lips), lingual control (tongue) and/or sequenced movements in order to achieve successful productions. I implement visual, verbal, auditory and tactile feedback across varying degrees and fade cues as we move forward in treatment to achieve independent productions.
My parents and I will sit and outline personal goals for their family --how they wish their child's communication can improve their family life and autonomy. Parents are welcomed to sit in our sessions and I provide tips and strategies with take home activities/suggestions after every session. We video tape sessions and have parents periodically review with their family to determine how they perceive intelligibility is increasing. We also provide awesome tailored 'CORE word lists' where we have parents bring specific books they read for their child and 'make them over' so that the child's target list is included in a functional and parent accessible way.
AAC has been a great way to incorporate high levels of auditory bombardment or oral feedback during session with my clients. We have used NovaChat, Sounding Board and PECS to aid in children's communication and have found that it boosts their ability to initiate programming their movements for speech purposes.
