My therapy approach for CAS revolves around the principles of motor learning and the importance of getting a high number of accurate productions during each session. I usually use the Dynamic Temporal and Tactile Cueing (DTTC) approach as it has the most research and evidence to support it and I have seen a lot of success with this method. I always involve the parents in choosing targets for therapy as I believe it's extremely important to choose words that are going to be meaningful to the child and used frequently in their everyday environment. The DTTC approach follows a progression from providing the most support (slowed simultaneous production) to the least amount of support (spontaneous production) so that the child has the best chance of producing the target correctly and feeling successful. I gradually reduce the amount of support as the motor plan for the target becomes more established until the child is able to produce the target on their own. At the beginning of therapy we do a lot of frequent productions in a row of the same word (massed practice) and as the word gets easier, we start to alternate between different words (distributed practice) to promote generalization. I use fun and engaging activities to keep the child motivated throughout the session and find ways to integrate the speech target words into the activity so that the child can practice their targets in meaningful contexts. My sessions are also heavily focused on supporting the parents as well. I believe parents are the key to success with apraxia therapy. I focus a lot of energy on ensuring the parent has all of the tools they need to practice their target words at home successfully with their child. I also provide the opportunity for my families to submit videos of their practice to me between sessions so that I can provide coaching and feedback and answer any questions they might have.
Parents are heavily involved in my therapy process. My company has developed a model wherein the family has the opportunity to submit videos of their home practice sessions with their child to me so that I can provide coaching and feedback to ensure the practice is being done correctly. Parent education and coaching is a huge component of my therapy process given the importance of FREQUENT and ACCURATE practice of targets for children with CAS.
I always make AAC a component of my therapy to help the child have another means of communicating when they are not understood verbally. I ensure that the child has access to the appropriate amount of vocabulary to allow for their language to grow. I have used both low and high tech with clients depending on the client's needs and the family's preference.
