Thank you for donating a Library Kit to your local library. For $30, the Library Kit will be shipped directly to the U.S. library of your choice. You will need the shipping and contact information of the library to complete this form. You will be notified via email when it is shipped.

- 1 copy of “CAS What’s That” flyer
- 1 copy of “Parent Portal” flyer
- 10 “New to Apraxia” brochures
- 10 Apraxia Kids Bookmarks
- Activity pages – 1 coloring page and 1 word search to keep on file for making copies
