I first establish motivation by building rapport with a child. The way that I do this is by pairing myself with highly preferred toys and activities in the first few sessions. I use the Kaufman Speech to Language Protocol (K-SLP), which is based upon principles of motor learning. My treatment procedures include using multisensory cues, fading cues gradually, errorless teaching (cueing before failure), error correction, and mixing and varying tasks to avoid overgeneralization. After I choose targets for a child based on their initial assessment, I use a multi-sensory approach to elicit those targets from a child. This may include using hand/gesture cues, explanation cues ("lips together"), and other verbal and visual cues throughout our sessions. As soon as I can, I start embedding targets into highly preferred play activities. This is to help the child maintain the motor plan of their speech targets in a functional way.
I enjoy coaching parents on how to work with their child in a naturalistic environment. The bottom line is this- parents do not need to imitate what I do in therapy sessions. Instead, they need instruction on how to help their child communicate effectively in their everyday activities. In addition to having parents watch my sessions, I provide a target list and home programs with specific instructions on how to use targets during daily activities. I highly recommend using daily routines to work on speech. This can be during bath time, while brushing your teeth, or during mealtimes. The more functional, the better.
I have used sign language to facilitate spoken language as well as to help a child learn new vocabulary and request successfully. Sign language is used as a "bridge" to vocal communication in my treatment sessions. If a child is a good candidate for a high tech AAC device, I refer to a speech-language pathologist that specializes in AAC for an evaluation and follow their lead on recommendations.
