For younger children or children with more severe motor planning difficulties, dynamic temporal and tactile cueing (DTTC) is used. As children are older and more verbal, treatment advances to differing approaches depending on the child’s motor planning abilities and any other co-occurring speech sound disorders (such as phonological disorders). For an older child that is using consistent phrases and sentences, Rapid Syllable Transition Training (ReST) may be used. For a child with a co-occurring phonological disorder, treatment would incorporate phonological treatment approaches.
Continued assessment is completed during ongoing therapy to assess the child’s progress of treatment and aid in clinician decision making for plan of care moving forward.
I feel strongly that parents should be included in the therapy process, both observing and understanding what occurs during therapy sessions and then also at home to help promote generalization, when recommended.
Need and use of AAC is considered specifically for each child. Gesture use is supported to help increase understanding and communicative effectiveness from the beginning of treatment. Sign language may be recommended as well if a child is responding well to models. Use of low tech and/or high tech AAC is implemented, as needed. Initial goals of AAC are to provide a bridge to communication while continuing to work strongly on verbal communication/speech. The use of complex AAC systems may be deferred for a specific child if they are responding well to treatment and making good progress with verbal communication.
