My first priority when treating any communication disorder is to build authentic connections with the child and their family, and create a positive environment of mutual trust. When the child has apraxia, I integrate the information I have gathered through relationship building, parent interview, assessment data, and trial therapy to match the child to the right motor-based treatment for their current skill level, readiness to participate, and communication priorities. I continuously monitor a child’s response to treatment and am not afraid to make adjustments, learn something new, or collaborate with other experts to determine the right next step. The treatment approaches I use most include DTTC, ReST, Speech Motor Chaining and elements of PROMPT in addition to individualized, dynamic motor interventions based on the principles of motor learning. Apraxia therapy involves hard work due to the high number of repetitions and trials required to make progress, but it is always my goal to ensure sessions are enjoyable and that we are incorporating targets into play and natural communication opportunities whenever possible.
Parents/caregivers are encouraged to attend each session to stay informed on the therapy approach that is being used, learn and practice cuing strategies, do’s and don’ts for home practice (and why!), and to discuss progress and goals on an ongoing basis. If caregivers are unable to attend, I am happy to communicate through written updates and/or routine conferences.
If a child enters therapy without a reliable way to communicate that needs thoughts, and ideas, my first priority in therapy is establishing AAC, whether that is sign language, low-tech picture communication, or a high-tech speech generating device. Their AAC system will continue to be modeled, incorporated, and honored as a part of their total communication throughout therapy while we work toward improved speech production.
