During initial sessions, I focus on building rapport and earning the child's trust. I model words and expand utterances as much as I can without expectation that the child will imitate my productions. At this time, if the parents and child do not have a reliable way to communicate with each other, I suggest printing pictures of common wants and needs the child has and hanging them in an easy-access place, such as the refrigerator. More parent education is given at this time as well depending on the family's needs. As sessions progress, I continue to use play-based practice and slowly begin to work in moments of structured practice. Structured practice then consists of repetitions of target words/syllable shapes/motor movements, and I rely heavily on visual, verbal, and tactile cues. I begin with heavy cueing and slowly fade as their accuracy increases over time. Breaks are offered as needed in order to maintain morale and motivation!
Parent involvement in CAS therapy is critical to maximal success and generalization. I love having parents provide a list of words that are important to their family and their child that I can target in therapy in order to make sessions as functional as possible. Parent education is a passion of mine and I enjoy teaching parents/caregivers how they can best help their child progress and communicate at home.
AAC has been used among my CAS patients on a case-by-case basis. I have utilized both high tech and low tech systems, ranging from a simple picture system to programs such as Proloquo2Go and LAMP. I enjoy finding the best way to help children communicate functionally before verbal speech is a refined form of communication.
