My therapy protocol for CAS is based on the principles of motor learning. In early treatment, I work with the family to identify a daily routine or activity that is interesting and motivating to the child and plan treatment targets that are functional for the routine and within the child's sound and pattern inventory. For example, if the motivating and engaging routine is playing on the swing set, and the child can produce CV, VC patterns and has /p, t, m, n/ in her inventory, I would target "up, no, me, pu(sh)" for an initial set. I would use as many cues (modeling, showing, gesturing, touch cues, etc.) as necessary to elicit as many accurate productions of the target words the child will produce during the routine. I offer specific feedback for each trial. I fade the cues as quickly as possible so that the child is producing the patterns as independently as possible.
I provide all of my treatment in the client’s home and encourage parents and caregivers to be involved. I work with the parents and child to identify vocabulary and phrases that are important to the child and relevant to daily routines in the home. I model cueing and feedback techniques so parents can continue practice when I’m not present. I want parents to understand the goals of treatment and feel empowered to advocate for their child.
I have used both low and high-tech AAC with children with CAS. I have created many communication boards for families to use as part of their daily routines, using both line drawn pictures (Lessonpix, for example) and real photos of objects and people from the child’s life. For example, I recently created a fringe vocab board related to JoJo Siwa, using Google images, because I have a client who really enjoys watching her videos and wanted to be able to engage with friends about the topic. I almost always introduce a core vocab board (either paper or digital) early on in treatment and model, model, model its use for my students and their families. I recently trialed communication apps for iPad with a family.
