Childhood Apraxia of Speech (CAS) is a disorder of motor planning and programming that requires specialized treatment using a motor-based approach. This helps children learn complex motor control needed to produce sounds, words, and sentences. Treatment includes frequent and intensive practice, focus on movement (whole words rather than individual sounds), multisensory input (verbal/visual/tactile cues), carefully chosen target words and types of cues individualized to each child, and a focus on prosody (intonation, rhythm, and stress). There are several evidenced-based treatment approaches that have been found to be effective.
While the specialized treatment we as speech-language pathologists provide is crucial, parent involvement is the key to generalizing skills into the real world. I include parents in sessions periodically to learn strategies for carry-over. I recommend modeling the correct production of a word/phrase/sentence, pick a dedicated time to practice at home with lots of repetition, keep a positive mindset, and respond to all forms of communication (i.e. gestures, signs, speech, pictures) that your child uses. Additionally, I recommend incorporating target words using in therapy into the home practice tasks. I encourage parents to be involved each and every step of the way!
Functional communication comes in many forms including, verbal speech, gestures, AAC devices, pictures, etc. If a child demonstrates non-functional communication due to the severity of their CAS, I encourage the use of multimodal communication, including an AAC device (high and/or low tech). Evidenced-based research proves that the use of an augmentative and alternative communication device will NOT negatively impact your child's ability to improve their verbal language and speech production. It helps decrease their frustration by providing functional communication and facilitates growth in expanding their verbal language.
