To treat a child with moderate to severe CAS, I would recommend 3-5 short sessions a week, as shorter, frequent sessions are proven to be the most effective for CAS. I would plan to start with a small group of simple words with a variety of consonants and vowels and different syllable shapes to focus on. I would discuss words with parents first, to choose personal/functional words for the child to add to their vocabulary - words that would be useful to the child and family. I use a combination of verbal, visual, and tactile cues to assist children in being successful, as well as immediate, specific feedback to increase child's knowledge of accuracy/errors. As time goes on, feedback and cueing would decrease and randomization of practice would increase to help children climb the ladder of progress and generalize practice to the real world. And of course, I would work with parents to find motivating and exciting ways to work on this plan with their child.
Parents are always welcome to observe all or part of sessions. Parents are required to join at least the last 5-10 minutes of session in order to discuss at home practice as well as learn visual and verbal cues used in therapy sessions.
I have used some low tech AAC (pictures) to help children communicate in early intervention - often times parents and children can become very frustrated when the child is unable to communicate wants and needs effectively - pictures and low tech AAC helps significantly and gives families targets to work on.
