For children with moderate to severe CAS, my approach focuses on motor skills and motor learning. I usually begin by improving the child's attention and effort toward movement of non-verbal productions (ex: opening and closing mouth, movement from smile to pucker, etc.) For a child that is able to produce certain sounds, I focus on those sounds already in their repertoire to add new sounds in order to create functional words. I incorporate activities that are fun and engaging (games, sticker charts) but that are intended to keep the child's attention to my face for maximum visual feedback and maximum repetitions. My feedback includes visual, verbal, and tactile cueing with fading of cues as needed. For my severe CAS kids, I usually try to schedule sessions at least 2x/week for 30 minutes.
I encourage parents to be present during my therapy sessions so that they can observe and provide/retrieve feedback in regards to progress and ongoing treatment plans or goals. Once I feel that parents are confident and knowledgeable of treatment goals and strategies, I provide activities to work on at home.
I have used low tech AAC such as pictures and high tech. such as proloqou2go to assist kids who demonstrate severe CAS in order to provide supplemental supports for communication.
