In treatment, I tailor each session to the child's specific goals and offer activities based on his/her personal interests to keep them motivated! With parent/guardian input, I select functional and meaningful target words based on the motor speech hierarchy. I consider a child’s control of phonation (voice), mandible (jaw), labial (lip)- facial, and tongue muscles, as well as prosody (melody) when selecting targets. Therapy is based on the principles of motor learning and focuses on speech movements in varying syllable shapes. I want children to experience communication successes and provide multi-sensory supports to achieve this. PROMPT and DTTC are the primary treatment methods I use. When needed, I also implement and work with augmentative communication methods such as PECS (Picture Exchange Communication system) and speech generating devices to facilitate effective communication.
Parents are an important part of their child's treatment on various levels from providing input on goals and target words to helping generalize successes outside the therapy room. For children in preschool or younger, parents often participate in initial treatment sessions with ongoing discussion of strategies and supports that are being used. As a child becomes more comfortable with separating from caregivers for treatment sessions, parents are given feedback on each session verbally or in written form with suggestions on how to support treatment at home. I like to collaborate with parents/ caregivers to understand important words or phrases that would have an impact for their child in their daily life.
I use AAC with my clients for various reasons. When young children present with frustration in verbally communicating their wants and needs, I typically introduce a picture exchange system early to reduce frustration and increase effectiveness of their communication attempts. I also use augmentative communication systems, both low and high-tech to work on expressive language skills and as a primary means of communication when verbal communication is challenging.
