We are so used to treating articulation disorders because they occur so much more often than CAS, that it is very important for all of us (SLPs) to separate our way of thinking and planning. We SHOULD NOT plan to target a specific consonant such as in the initial or final position of words. We SHOULD NOT be strict about following a sound development chart or following a particular sequence of consonants that are introduced. We have to be very receptive to what the child is showing us he's ready for. And we SHOULD NOT have a long list of words to practice during therapy or to only allow a couple of trials per word. For example, I've had sessions with CAS children where we practiced 1 or 2 words the entire therapy session. But we practice each word at LEAST 50 times. We have to remember that this IS a motor planning speech disorder, so a child with CAS needs to practice a target word many, many times to allow that neuromotor pathway to form and become more of an automatic production for them.
They are very involved. They are present in the session (at-home therapy). I explain what I'm doing during the session and why I'm doing it. I invite them to try and give parents an opportunity to prompt/cue child during the session so that they build confidence in continuing treatment plan when I'm gone. I also provide them with activities to do with their child and we will begin the next session by talking about that activity with the child.
I have used low tech and as simple as having a visual schedule to help the child transition between tasks and decrease negative behaviors. I also teach them simple signs. I've also made communication books with picture symbols of familiar items in home so that they can keep it at home. I have also used ProLoQuo on my iPad to teach parents about it and so that they see how it works and what we can achieve with it before they decide to purchase it themselves. I've seen them graduate from their AAC devices and that is a big milestone for parents and kids.
