I typically use a combination of treatment techniques. I often use the Kaufman Kits in combination with other approaches. I complete drill work (e.g., using the Kaufman kit cards) in combination with play based functional treatment (e.g., having the request "I want to eat two cookies". I teach the child appropriate word approximations that allow them to communicate in a functional way. I focus on increasing the child's phonetic inventory while using those phonemes within different vowel-consonant combinations. I progress the child to phrases and sentences as quickly as possible. In combination with the Kaufman Approach I also use other cues/approaches such as PROMPT, hand cues, immediate feedback, and simultaneous speech. I fade cueing as soon as possible so that the child is producing the word/phrase independently. Treatment sessions provide intensive practice of phonemes/words/phrases so that the child is producing as many trials as possible. I also incorporate language goals into our sessions as well (e.g., saying basic concept words such as up, down, colours, etc.) to ensure the child can be the best communicator they can be. If appropriate, I will also work on using AAC with the child. I work closely with the child's parents so they can continue to make progress at home.
Parents always attend therapy sessions. I typically have the parent just observe to begin with. Once the parent has seen me work with the child for a while I will have the parent join in on our session. I will instruct the parent on how to play with their child in a way that will provide many opportunities to practice speech targets. I provide them with feedback during that time telling them what to do differently and what they are doing well. At the end of each session I provide them with opportunity to ask questions and I discuss what we did, and what they can continue to do until the next therapy session. I provide them with ideas how they can incorporate speech practice into every day activities and provide them with opportunity charts to track practice if appropriate.
I often need to incorporate some form of AAC into therapy sessions for the children who have CAS. I have used low tech such as sign language, using choice boards, and PECs. I have also used high tech AAC such as proloquo2go. Most of the time I use it as a form of communication when communication break downs occur. When teaching the child how to use AAC I often have them use the AAC and then if appropriate, say the word(s) verbally to encourage verbal speech as well as provide them with extra practice.
