Initial treatment plan decisions would be made after a dynamic assessment. I would begin by assessing the child's current phonemic repertoire and consider the syllable shapes they already use. There are a number of decisions I would need to make when devising initial stimulus set that are directly related to this idea of targeting movement gestures–especially important to vowels. Initial stimuli would be chosen based on vowel errors noted in the motor speech exam as well as syllable shape. If a child's CAS is severe, I would be careful to restrict to only two new vowels (distorted vowels) across a couple of co-articulatory contexts and have at least 2 or 3 syllable shapes in initial stimuli. I would implement Dynamic Temporal and Tactile Cueing (DTTC) for severe CAS.
Parents are involved the therapy progress by observation of the therapy sessions, direct coaching and video modeling of carryover activities for individualized home exercise plans.
I have used multimodal communication-both high and low tech AAC for children with severe CAS simultaneously with speech production work so that they are able to communicate effectively and continue develop their expressive language skills. Every child deserves a voice. Adding AAC will support speech and language development, not limit it.
