My therapy for a child with CAS changes over time and varies with severity and progress. Of course, my approach will be individualized for your child and will consider their specific needs. Therapy is based on the principles of motor learning, focusing on movement and the transitions between sounds, syllables, and words rather than individual speech sounds. Typically, therapy will start with an assessment of speech and language skills, including a dynamic assessment of their motor speech abilities. Early in treatment and when the apraxia is severe, sessions are more frequent and often shorter in length, depending on your child’s ability to attend and imitate. Together, we will select a small set of targeted words that are functional and motivating, keeping in mind what the child can already produce. Our goal will be a high number of repetitions giving support and feedback. As the characteristics of apraxia diminish, a more traditional approach may be introduced to minimize any lingering speech sound errors.
The keys to successful therapy are consistency and parent participation. Parents are encouraged to share key vocabulary (e.g., family names, etc.) as well as the child’s interests, dislikes, and motivators to help make target selection meaningful and to help me develop rapport and trust with the child. Parents are welcome to observe and or participate in sessions and are asked to help in the carryover process by practicing words and phrases that the child has mastered in therapy. I will work directly with parents to show them how to cue and support their child in the generalization process at home. Parents are their child’s experts and advocates, and those roles are always respected. We are a team that is there to support and help your child more effectively communicate.
For all children, the most important goal is communication. This may include low or high tech AAC that can help them to better participate in the classroom, get their needs met, and continue to increase both receptive and expressive language skills. I have worked with families and teachers to make sure everyone is on the same page and using the AAC system to help the child communicate, fading its use as the child is better able to orally communicate.
