Often children who are late talkers can show some indicators of suspected CAS early on. I work on a variety of motor imitation in a child-directed play environment. As your child starts to have more words/sounds (whether in imitation or spontaneously), I work on increasing your child's confidence to produce those words in a variety of settings. While your child continues to build confidence in the sounds/words they can produce, I also work on combining those sounds into new syllable shapes or with new vowels, etc. I often follow principles of Dynamic Temporal and Tactile cueing, in which the child and I say the word together, often with visual or tactile supports. I also incorporate principles of Integrated Phonological Awareness, especially for children who are interested in letters. I occasionally incorporate the use of Rapid Syllable Transitioning Treatment for children who have high awareness of their inaccurate productions and low confidence around speaking. As time progresses, I reduce my models/cues and encourage your child to produce words spontaneously. I always praise and reinforce any attempt at verbal communication, especially early on. I often incorporate other early language techniques, such as recasting (repeating back what they say so they know their message what understood) and expansions (adding another word to what they say). I also incorporate early sign language or the use of AAC devices in treatment to allow your child to communicate their wants/needs now and continue to expand their current language skills.
I encourage all families to attend and participate in sessions as able. I encourage families to be part of the therapy session by having them model words and give encouragement throughout the session. I always provide families with home programming strategies, whether it is encouraging signs or AAC use at home, practicing a successful word from the session, or modeling animal sounds. I always check in with families to see if there were any new words or skills at home that week and then try to reinforce those in the session, build on those reported skills, and provide new strategies to continue growth/success at home.
I have used a variety of alternative communication methods to support language expression in kids of a variety of ages, from early signs at ages 1-2 years, to advanced tablets from children ages 2-18 years. I always have the family give input on their preferred mode of communication, as well as trial different modalities and show families the child's successes with different modalities so they can make the most informed decision for their child and family.
