For children with a diagnosis or suspected apraxia of speech, I recommend frequent and intense individualized therapy sessions (2-3 days per week, 30-45 minute sessions). Emphasis is placed on improving motor planning and programming, including repetitive planning, programming, and production practice for more intelligible speech. I try to obtain as many correct productions as possible of a single syllable/word/phrase within each therapy session. Alternative augmentative communication devices may be used in conjunction with speech production to ease the frustration of the child until he/she becomes more intelligible. Appropriate prosody is incorporated into each therapy target. I am PROMPT+ trained and also incorporate the Dynamic Temporal and Tactile Cueing (DTTC) system and Kaufman Speech to Language Protocol materials along with multi-sensory cueing methods into therapy sessions.
Parents are encouraged to observe all therapy sessions and attempt therapeutic techniques at the end of the session (if client has achieved success during the session with speech targets). Homework and suggested activities are given to aid in generalization at home.
I've used PECS, communication books, Dynavox devices, and iPad apps as a way for children to communicate their wants and needs without frustration until speech becomes more intelligible. I try to fade AAC as the child becomes more intelligible.
