Begin with the speech sounds the child is already stimulable for in V and CV productions. Concentrated, repeated production of the sounds, first imitated and then spontaneous, using auditory, visual, tactile, and cognitive cues. We can then work on incorporating those productions into a variety of contexts as the child gains confidence in his or her production of those targets. Eventually, we can increase the length of target words. Homework includes frequent and intensive practice of the target productions. Simultaneously, we can work on introducing additional sounds into the child's repertoire of pattern productions using a variety of cues, then repeating the above process as the child gains accuracy with those sounds. Goals may also target prosody and rate of speech and augmentative communication, if necessary.
I provide in-home, family centered speech/language therapy. I encourage parents to sit with me during the speech/language therapy session and participate in all activities. Parents are provided with specific activities to use during the week with their child. Parents are actively involved in all aspects of the process, from the initial evaluation, to setting therapy goals, to practicing in-between sessions.
I almost always recommend beginning sign language to children with limited verbal productions so that they can communicate their wants and needs in the short term. I have also used picture communication, both on paper and computer, for older children with limited expressive communication skills, in conjunction with verbal therapy.
