Therapy is guided by principles of motor learning: Child is always encouraged to watch therapist's face directly or while looking in a mirror, in a "look, listen, do what I do" approach. Therapy addresses a small number of targets, with large amount of practice for each target, touch cues are often used, with verbal placement instructions appropriate to the child's age, with frequent feedback regarding accuracy of speech movements early in the intervention process, then faded as therapy progresses. Therapy targets increase in complexity over time, with success on previous targets. Always, functionality of targets is considered, as well as linguistic and phonetic complexity. Parents are included in at least a portion of all sessions, so they can provide intervention in the home on a daily basis in more natural contexts. In severe CAS, simple language boards for functional communication are often used. PECS is offered as an option for functional communication in some cases, as well as a variety of iPad communication apps.
Parents attend all or a portion of all therapy sessions. They are shown tactile and visual cues that are effective with their child. They are provided an explanation of how targets are selected, and for their input regarding functional words in the home setting that could be targets of intervention.
I have attended PECS workshops and LAMP training, as well as a variety of general AAC seminars. If a child does not have functional speech, then AAC is introduced early in the intervention process. Our clinic has purchased iPads with a few AAC apps that we use as loaners to families, as needed. Depending on the specific needs of the child and the family, an AAC method is determined. We continue to work on speech at every session, and provide training for the AAC method as well.
