I begin by targeting speech sounds indicated by the formal and informal assessment. I typically start with CV an VC functional words. I utilize direct imitation, mass practice and specific feedback in the beginning. Once they are 80% or higher with productions I fade prompts and use delayed imitation. I then increase complexity by using target phonemes with in cvc and cvcv words. I place phonemes that the child is successful with at the initial, medial and final word positions to vary complexity and diversity. I use functional words that the child is successful with and place into predictable sentences (e.g., I see a _____). I make sure to continue to increase complexity by using a variety of vowel and consonant combinations. I target various prosodies with target words from the beginning and into structured conversation.
In the beginning of therapy I often also include low or high tech AAC, depending on the child's needs, in order to build receptive and expressive language.
During the evaluation process and the beginning of treatment, I provide education on CAS and therapy targets. I take data on functional words and send word and sentence cards home (if they are 80% or higher with words). I encourage parents to observe at least one session and implement the coaching model in order to increase effectiveness of daily practice at home.
I encourage children to use multiple modalities (e.g., pictures, gestures, sign, AAC) to communicate their wants and needs. I have used high tech devices (e.g., dynavox with Proloquo2go) with students in order to build language and use for functional communication. I've also used low-tech AAC (PECs, communication books) for students to use in class and in the community. It's important to give the child a way to communicate throughout their day in order to express wants and needs. Research also shows that children are more successful when AAC is paired with speech sound tx.
