Speech therapy for a child with CAS is highly individualized. I take each child's skills, weaknesses and interests into account. I focus on the "movement" of speech, which includes a variety of mutli-sensory cues to help the child learn and recognize the motor plan behind saying a target word. Different cues I use include touch cues on a child's chin, mouth or cheeks, gestural cues to indicate where our tongue or mouth should be positioned for that syllable structure and visual cues using my mouth to help the child sequence their mouth's movements. I like to keep therapy fun and motivating so that each child learns that communicating can be fun and rewarding!
I like to have parents very involved in sessions depending on the child. Some children do best with their parents out of session. In these cases I debrief with parent for a few minutes and provide strategies and work to practice at home. I have previous work experience in early intervention, which emphasizes the PIWI model (parents interacting with infants). This model emphasizes the importance of carryover at home during daily routines, a child can only be in therapy so long each week so it is important for me that the parent understands the strategies that are working in session so that they can carry them over for continued practice at home. It is also important for me that I understand potential areas of difficulty at home in terms of the child's communication breakdowns to better assist the family communicate with each other.
I love to introduce and implement AAC into my practice with children with CAS. I use a variety of low tech and high tech AAC and underscore that the most important goal is that each child has a voice and a way to communicate with peers and adults. We use a form of AAC to communicate daily with others (I.e. facial expressions, gestures, texting, writing, etc.) it's important to remember that we need to give children every opportunity to use their voice and communicate even when we are working simultaneously on speech production.
