My treatment approach involves using the phonemic inventory from the assessment and stimulability testing to create core vocabularies around preferred activities that also match targeted word shapes. The goal is to use functional, motivating words to practice targeted mouth/articulatory movements like air flow, voicing, jaw grading, tongue movement or nasality. The therapy session is based on motor learning principles of repeated, correct productions with specific feedback on performance and short, frequent sessions. When productive, same targets are used in distributed practice and moved to home program. Cueing strategies used are tactile cues, in unison speech and fading of cues as quickly as possible within an activity to reduce prompt dependence. Once productive, using targeted words in various activities and moving into phrases and sentences to promote generalization. Other strategies involve using rhythm and music to develop and improve prosody and intonation. Also, sign and/or AAC is used along with speech strategies to encourage multi-modal communication. Ongoing involvement and communication with caregivers and an updated home program for carryover to home, school and community. I have DTTC and PROMPT training which I incorporate into my CAS evaluation and treatment plan.
They give detailed history, sit in on virtually all sessions, practice modeling and cueing after observing a while and I coach them, learn to model and promote use of talker at home and/or learn, practice and use signs. I have implemented or used picture systems, various iPad devices, recently went through LAMP training and use with two of my clients with autism and CAS diagnosis. I incorporate sign to support comprehension and facilitate communication with virtually all of my clients, especially with CAS and I educate caregivers and teachers on the importance of total communication.
